Haemorrhoids, the golden vein, are observed in 60% – 90% of the population. But only 50% of patients will admit to this problem. Up to 70% of patients who have varicose veins on lower limbs or some form of chronic venous disease also suffer from haemorrhoids. The haemorrhoid, the golden vein, is actually a bulged vein under the mucous membrane in the area of the mucous membrane of the anal canal. Rectal polyps and condylomas are growths on the mucous membrane of the anal canal or rectum and are a common disease. They cause various unpleasant sensations and symptoms, such as itching, perianal eczema, bleeding from the anal canal, pain at the site of the polyp, sensation of a formation coming out during bowel movement and bowel urgency. These symptoms can be confused with an advanced form of haemorrhoidal disease, so it is important to diagnose them thoroughly and distinguish them also from malignant tumours of the rectum in the anal canal. The samples for histology can be collected during rectoscopy or surgically in the outpatient clinic under local anaesthesia. In addition, a colonoscopic examination of the entire colon at gastroenterological outpatient clinics is recommended to evaluate the overall gut health and to detect possible additional polyps in the colon. Condylomas are a different type of growths, and we treat them with laser, arrange histology and solve them in collaboration with dermatologists and immunologists.
Anal tear, known as fissure, is accompanied by severe pain during bowel movements, bleeding and anal spasm. The causes include hard stools, constipation and strenuous pushing during bowel movement. We distinguish acute and chronic fissures, which can be complicated by the occurrence of polyps. Treatment includes conservative methods, such as topical ointments and dietary changes, and when conservative treatment fails, we perform outpatient surgeries under local anaesthesia using radiofrequency extirpation or laser treatment.
Itching of the anus is caused by eczema, in which the skin and mucous membranes of the anus are irritated. Symptoms include itching, discomfort in the rectal area, a feeling of a wet anus, blood on toilet paper when tears occur at the junction of the mucous membrane and skin of the anal canal. This itching can occur in association with various diseases of the rectum, such as polyps, haemorrhoids, fissures and fistulas. It can also occur in inflammatory bowel diseases (ulcerative colitis, Crohn's disease), cancer and other health conditions. Itching of the anus is usually not caused by poor hygiene, but rather by excessive hygiene and the use of products that can irritate the skin.
A fistula is a small canal or opening around the rectum, through which stool or pus can leak. It causes discomfort and restricts patients' hygiene because it irritates the skin. The cause is often a perianal abscess caused by the inflammation of the outlet of the anal gland. The diagnosis includes clinical examination, rectoscopy, colonoscopy and other examinations. Surgical solutions, such as dissection, elastic ligature and laser FiLaC surgery, are performed on an outpatient basis under local anaesthesia and allow for rapid recovery with minimal complications. In the Proctological Outpatient Clinic, we also examine various unpleasant sensations and pain in the rectal area and search for the cause. It can be the aforementioned diseases of the rectum and also pelvic floor disorders, such as prolapse, incontinence, or urological and gynaecological causes.
Anal duplications are loose skin growths from the anus. They can occur, for example, after thrombotisation of haemorrhoids, in long-term untreated haemorrhoids, after difficult childbirth, but also in chronic anal fissure. Their presence can be painful and require surgical treatment, which is performed on an outpatient basis under local anaesthesia. After a thorough proctological examination, a radiofrequency scalpel is used to minimise damage to the surrounding tissues, which means less swelling after the procedure.
Thrombosed haemorrhoids, also known as painful golden vein nodules, cause sudden pain and nodule formation in the anal area. When ignoring it, more massive bleeding may appear on Day 3–4. Recurrent inflammation causes swelling and anal duplication. Early diagnosis and treatment will prevent complications. In case of thrombosis, it is important to see a doctor within 3–5 days for painless removal of the lump.

The doctor uses a thin disposable rectoscope to visually examine and explore the inner parts of the rectum up to the lower part of the colon – the so-called sigmoid colon.
An elegant, fast, painless method, without impairing the integrity of the anal canal or sphincter apparatus. The treatment consists of up to 3–4 sessions, and 80% of cases up to Grade II can be treated by "rubber band ligation". In higher grades, surgical treatment is preferable.
Very gentle outpatient treatment with minimal post-operative pain. The aim of the procedure is to ligate the feeding vessel of the haemorrhoid to stop the blood flow at the site, and the haemorrhoid is then excised using a radiofrequency scalpel.

Rafaelo – radiofrequency coagulation is one of the advanced gentle methods of removing haemorrhoids where the heat source is radiofrequency waves; it is performed under local anaesthesia without hospitalisation.
Laser haemorrhoidoplasty – is one of the advanced gentle methods of haemorrhoid removal where the heat source is a laser – light thermal energy; it is performed under local anaesthesia without hospitalisation.
Fistula surgeries are surgical procedures that are performed to remove anal fistulas, which are abnormal canals between the rectum and the surrounding tissues. One of the advanced methods of fistula closure is FILAC where the heat source is a diode laser. If the heat source is radiofrequency, the technique is called FISTURA. Both techniques are preceded by a thorough examination and insertion of a drain for a minimum of 2 months until the abscess has drained off and fluid production in the fistula is minimal. The success rate of the techniques is about 70%.
The basic principle of fissure treatment is the removal of the rigid fibrotic edges of the fissure. A radiofrequency scalpel is used to remove chronic fibrous tissue that is preventing the healing of the fissure. The bottom of the fissure is either treated with a laser or with suturing. For large fissures, we add a technique to relieve the tightness of the rectum called sphincterotomy, where the sphincter is incised so that incontinence does not occur. With the current modern use of laser, sphincterotomies are being discontinued as the laser method is gentle, shortens healing time and reduces post-operative pain.
Fistula surgeries are surgical procedures that are performed to remove anal fistulas. Elastic ligature is a basic technique to empty a perianal abscess. A special silicone drain is left in place for at least 2 months until the abscess cavity is drained off and we wait for the reduction of fluid secretion from the fistula.
Statistics show that more than 60% of patients have rectal problems and ¼ of patients come late for examination in Slovakia. All you need is to undergo a painless outpatient examination – a rectoscopy at the Proktovena centre where, after the diagnosis is made, the treatment is painless and without hospitalisation.
Do not underestimate the preventive examination of the rectum – a rectoscopy, during which we can detect and then remove early stage colon and rectal cancer on an outpatient basis.
Obesity is also one of the factors that can cause haemorrhoids to develop or worsen more quickly.

Many factors influence the development of a golden vein, and one of them is lack of physical activity. But not all sports are suitable for proctology patients. However, it is good idea to buy a yoga class, running sneakers or trekking poles.
Drink plenty of fluids, about 2 litres a day, eat a non-irritating diet rich in fibre, try to have bowel movements regularly, do not lift heavy things, use irritating cosmetics for hygiene, wear tight clothing, eat chilli and try to avoid cycling in the acute stage.
